Loss or Theft of Cards

To report your Signature ID card as lost or stolen:
- log in to Route Y
- select the Signature Card and Meal Plans link from the Miscellaneous drop-down menu
- select the Lost/Stolen Card link under the Personalize section on the right of the web page
- click deactivate
If you find your card, you can active it by following these same steps.
You can also report your card lost or stolen by notifying a cashier at any Signature Card location, or by contacting the Signature Card ID Center (2310 WSC) at 801-422-3866.
The ID Center will email you if your card is turned in - this can take anywhere from 2 days to 2 weeks.

To access Signature Card funds or a Meal Plan while your card is deactivated:
- log in to Route Y
- select Personal Information from the Communication drop-down menu
- select ID Card
- print - this is your temporary ID (If you are printing in a computer lab on campus make sure the lab has an attendant. To pay for the print job with your Signature card let the lab attendant know that you are printing a temporary ID and they can type in your ID number.)
Temporary IDs may be used to purchase goods and services at dining service locations, the Bookstore (3rd floor accounts desk ONLY), computer labs with attendants, and the testing center. Your ID number will be hand entered by the cashier - transactions which are hand entered will access your funds while swiping a deactivated card will not.
Temporary IDs are not accepted at Athletic facilities or the Harold B. Lee Library.

To get a new Signature ID card:
Visit the Signature Card ID Center in 2310 WSC.
There is a $10.00 fee to replace a card which has been lost or intentionally abused.
Stolen cards will be replaced at no charge after a report has been filed with the police (any police dept.). You must bring the police case number to the ID Center. Patrons falsely reporting a stolen card will be reported to the Honor Code Office.
Transactions made on a stolen Signature/ID card that has not been reported lost or stolen may be the responsibility of the card holder.
